Hardware ID - Hardware ID of a user to blacklist.

Region/State - Region/State to blacklist.

Country Code - Country Code to blacklist.

ASN Number - ASN Number to blacklist.

Enter blacklist data - The data you are blacklisting.

Reason - The reason for blacklisting.

Delete Blacklist - Deletes the blacklist entry.
